About the Organisation

Glenview is an aged care provider situated in Glenorchy, on the outskirts of Hobart and delivers a range of outreach support services to about 100 people living in their own homes. Outreach staff and clients work together to determine the amount and type of support that the client needs to remain living in their home as independently as possible. A range of services are available to all clients, including personal care assistance and helping with household chores, shopping and providing social opportunities so that clients can enjoy their lives to the full.

About the Opportunity

Glenview has an exciting opportunity for a number of Community Support Workers to join their Glenorchy team. Successful candidates can choose from full time, part time or casual/relief work.

In this pivotal role, you will be responsible for delivering all support needs as detailed in client care plans or as directed by the Community Service Manager, Case Manager or individual client.

Some of your specific responsibilities will include:

  • Delivering services to clients with complex needs;
  • Contributing to the day-to-day running of the Glenview Outreach program and services;
  • Providing relevant and timely advice to the Community Services Manager on all client support requirements including equipment, facilities and access to allied health professionals;
  • Monitoring and reporting changes in clients' physical and psychological status;
  • Liaising with health professionals, volunteers and service providers from other organisations, as directed by the Community Service Manager or Case Managers;
  • Reporting on all support services provided on behalf of the Glenview Outreach programs;
  • Identifying changing needs and/or priorities of clients and in conjunction with the Case Managers adapting services including times and locations to best meet clients' needs;
  • Working collaboratively with clients' families; and
  • Contributing to the professional development of the Outreach Department.

The successful candidates will hold a relevant qualification (Certificate III in Home and Community Services (Aged Care) or equivalent as a minimum) and will possess skills and experience in client-directed care, including personal care and related services, to the aged client group.

You will require well-developed communication and organisational skills, and the ability to work both autonomously and as part of a team.

Glenview is seeking candidates who can provide a National Police Certificate. You must have a current driver's licence, a clean and well-maintained vehicle available for work use, and be computer-literate.

Candidates with the ability to communicate in more than one spoken language will be held in high regard.
