About the Opportunity
Southern Cross has a fantastic opportunity for two (2) full time Registered Nurses / Care Assessors to join its team.
- Care Assessor working in the South West region of Sydney travelling predominantly around the Bankstown, Liverpool and Macarthur areas. The Care Assessor (Sydney South West) will work in conjunction with the Care Assessor team and assist with the assessment, monitoring and review of the Service User (the person receiving care in the community) needs.
- Care Assessor New Programs working and travelling (inter and intrastate) to establish new community care programs. The Care Assessor New Programs will work with the Public Affairs team to assist the Manager Care and Public Affairs in assessment and development of care plans/programs in the home.
Both positions travel throughout the community to undertake assessments, care planning and monitoring of care services in the community setting.
These are exciting and diverse roles that liaise with families, Service Personnel and external multi-disciplinary teams to assist people with disabilities (such as spinal cord injuries, ventilator dependent, brain injured and challenging behaviour) to live in their own home in the community.
The positions are full-time Monday to Friday, working hours are predominately 9am to 5pm, however, flexibility is required to accommodate training needs of Service Personnel and travel (including inter and intrastate for Care Assessor New Programs position).
Some of the more specific duties involved in this role include:
- Travelling about the area/s where service is provided
- Providing on-the-job training, support and guidance to Service Personnel
- Facilitating meetings to motivate, educate and assist service delivery
- Liaising with other health professionals in order to meet service delivery goals
- Working in conjunction with Admin Personnel to ensure service delivery needs and goals are met.
The successful applicant will have relevant qualifications and experience in Nursing (NSW Registration) and a proven understanding and/or experience working with people with disabilities and/or challenging behaviour. Community Care experience will be held in high regard.
It is essential you demonstrate excellent written and verbal communication skills, are able to communicate effectively with a wide range of stakeholders in a variety of settings, and have the ability to work as part of a team and independently.
The role would suit organised and energetic individual with a holistic approach to care and service delivery, the flexibility to meet the changing needs of the Service Users and who is willing to travel. Compassion, patience and the ability to implement change into systems and routines which are familiar to Service Users and Personnel alike are also required to fulfill these roles.
Successful applicants would need to be computer literate, have a current driver's licence and a reliable vehicle, and be willing to travel as the role involves assisting Service Users throughout the community in the areas advertised. Please note that all fuel and travel costs are covered by Southern Cross.
